Icons on menu items

For a while now gtk has stopped showing the icons that a few (very few)
commands have. The Directives->Add Rehearsal Mark command for example
has one (an A shape).
Is it just my installation, or is this quite general?
I ask because I will turn off the code that creates these until it can
be fixed...
These are less important now that we have translatable tooltips working.
