I was just reading this and noticed that I had the same problems that 
Amy had. However, my config was:

Title  Cell Online
URL    http://www.cell.com/
D      cell.com

So I added DJ for cell.com and that fixed the links for the pdf. So I 
wonder if Amy needs to logout or make sure to restart EZproxy again 
or something like that.

I am running EZproxy 5.0c.
