I'm not sure how mine "arrived", I expect it was in a letter from the 
tax authority to my first employer. I've certainly never had any direct 
notification (other than it appearing as a "reference number" on 
correspondence from time to time.

Yes, I have a standard US SSN which comes on a strange speckled blue 
piece of paper. Not sure of the exact weight, and whether or not it 
qualifies as "card".
