... which turned out to be due to a track on data line 13 in the raid
controller being eaten through by what looks like a pen mark, found
underneath an un-broken manufacturer placed label. ;(

The card's been repaired and soak tested which passed, so everything
should be back to rights now.  However, I will be scheduling the
replacement of this card in the very near future (early next week.)
