How about changing rules-edition part into something more "readable":

If [group] is [Hidden]
Then [deny] .....

The 'Custom' edit box is - for me - a bit confusing.
I guess your idea is to use combo box to choose subscriptions and
groups, but 'custom' editbox for jids.
How about making it only a combo box which would be choose-only when
selecting a subscription type and type-or-choose when it comes to jids
and groups?

One more thing: I don't know how it is on MacOS, but I'm not used to
see '+' and '-' buttons on Windows.
Honestly, it took me some time to understand what do these +/- buttons
in the bottom-left corner do.

This leads me to the last (yes, don't worry, it's the last one - I hope) point:
Most of people will probably use only one privacy list. And the list
of lists ;) on the left is a bit confusing: it takes much space, but
at the same time you don't know what does it really mean.

How about changing this UI to look more like status-presets options:

List of lists at the top, with 'Add' and 'Remove' (instead of +/-) on
the right, then rules-editor, then - at the very end - default and
fallback list, one below another.

And what about active list? I imagine choosing the active list in the
same way I choose my status: from a combo button in roster window.

Also, there could be a combo like this is 'Choose status' dialog. And
more: if we get advanced status presets (like in Remko's patch), how
about letting user to assing a privacy list to a status preset?
