[OT] Power over Ethernet (PoE) adapters - product suggestion

For power, my network device is PoE capable and also has a 12V/1A DC in.  
There are no power outlets near the device.  Also, the ethernet switch 
does not have any PoE ports.

I am looking for an adapter, that sits in the wiring closet alongside 
the Ethernet switch, with input a) Jack for DC12V/1A in, b) RJ45 LAN in 
(from switch), and output A) RJ45 LAN out (ethernet+dc power).

If you have used or know of such a beast please share make/model.
